Job Description

Hospice Weekend RN – Part-Time

VNA of Hanover & Spring Grove

Job Summary:
The Hospice Weekend RN provides professional nursing care to patients, families, and caregivers in accordance with the Hospice Medicare Conditions of Participation, Pennsylvania licensure laws, and agency policies. This position includes initial and comprehensive nursing assessments, development of care plans in coordination with the interdisciplinary team, and implementation and evaluation of individualized care. The role requires collaboration with physicians, patients, families, and caregivers to provide holistic end-of-life care.

Schedule:

  • Part-time weekend position
  • Saturday and Sunday only
  • 8- or 10-hour shifts (flexible)


Primary Responsibilities:
Nursing Process & Care Delivery:

  • Complete physical, emotional, and psychosocial assessments for patients and families.
  • Collaborate with physicians and interdisciplinary team to develop, implement, and revise individualized plans of care.
  • Provide symptom management, therapeutic nursing interventions, and patient/caregiver education.
  • Participate in IDT meetings and facilitate communication to ensure continuity of care.
  • Document care accurately and thoroughly in accordance with agency standards.

Education & Support:

  • Educate patients and families on disease progression, self-care, and medication management.
  • Act as a mentor and participate in training new hospice clinicians.
  • Supervise hospice aides and homemakers as required.
  • Promote professional development through participation in in-services and care conferences.

Resource Utilization & Compliance:

  • Manage time effectively and maintain productivity in accordance with agency expectations.
  • Maintain records per agency policy and ensure compliance with all regulations.
  • Operate within ethical standards and demonstrate agency values in all interactions.

Customer Service Excellence:

  • Provide compassionate, respectful care with a focus on patient satisfaction.
  • Foster positive relationships with patients, caregivers, and colleagues.
  • Represent the agency with professionalism and empathy.

Performance Improvement:

  • Participate in quality assurance and performance improvement initiatives.
  • Identify and report concerns impacting care delivery or outcomes.
  • Collaborate in reviewing clinical records for compliance and improvement opportunities.

Secondary Responsibilities:

  • Provide nursing support to the VNA Home Health department as needed.
  • Attend and participate in agency meetings, trainings, and team events.
  • Perform other related duties as assigned.

Physical Requirements and Working Conditions:

  • Communication: Strong verbal and written communication skills.
  • Mobility: Ability to sit, stand, walk, bend, kneel, and lift/carry up to 50 lbs.
  • Environment: Must be able to work in a variety of settings including patient homes and facilities.
  • Transportation: Valid driver’s license, reliable transportation, and automobile insurance required.


Qualifications:

  • Graduate of an accredited nursing program (BSN preferred).
  • Registered Nurse licensed in the Commonwealth of Pennsylvania.
  • 1–3 years of hospice experience preferred.
  • Current CPR certification.
  • Valid driver’s license and satisfactory criminal background check.
