They are looking to take on a Clinical Social Worker who will be responsible for conducting psychosocial assessments, developing treatment plans, and offering counseling and crisis intervention to patients facing medical and emotional challenges. This is a hybrid position based in Westchester County, NY.

Clinical Social Worker’s Responsibilities and Duties

• Provide emotional support, crisis intervention, and counseling to patients and families.
• Assess patients' psychosocial needs and develop individualized care plans.
• Collaborate with medical teams, case managers, and community resources to ensure comprehensive patient care.
• Assist with discharge planning and connect patients to appropriate support services.

Clinical Social Worker’s Qualifications and Skills

• Master’s Degree in Social Work (MSW) from an accredited program.
• Certified Social Worker in NY State (LMSW or LCSW).
• 2+ years of professional experience in a hospital setting.
• Strong communication, problem-solving, and advocacy skills.
